Maybe you could make an "Underwater" adventure that your creature has to get out of the water to eat, but as your creature has gills, it will slowly lose health outside the water (Lava pit effect?) and gain the health back when in the water.

Beautiful world! I love the creatures and having to avoiod the tribe along with predators! You should make more planets!
